學員須知 Notes
A. 運輸署會使用填報此表格上的個人資料作管理和監察此課程計劃及方便運輸署學員聯絡。
The personal data provided in this form will be used by the Transport Department (TD) for administering and overseeing this course and facilitating communication with the participant.
B. 學員在註冊表格上填報的個人資料或會向運輸署、政府其他決策局、部門及有關機構公開，作上文(A) 項所述的用途。
The personal data provided in the registration form may be disclosed to TD and other Government departments/relevant organizations for the purposes mentioned in Para (A).
C. 學員在課程中獲得的成績會向運輸署提供，以作紀錄。此外，學員所完成的習作或會向運輸署公開、以便核實成績。
Participant's course results will be provided to the TD for recording and verification purposes.
D. 若八號或以上風球或黑色暴雨警告信號於上課前兩小時仍未除下，當天之課堂將一律取消。本校會在稍後時間與學員聯絡，並重新安排上課日期。
All classes will be canceled if a No.8 Typhoon Signal or Black Rain Storm Signal is hoisted within 2 hours of class commencement. Students will be contacted for class rescheduling as soon as possible.
E. 課程報名後，駕駛改進課程 - 1個月內有效；
公共司機職前課程 - 4個月內完成。
After registering for the course, the Driver Improvement Course is valid for 1 month;
The Public Driver Pre-employment Course must be completed within 4 months.
F. 職前基礎班及專屬課程需要同一個中心完成。
The pre-employment foundation class and specialized courses need to be completed at the same center.
G. 運輸署需時評核期末試卷及將成績交回職前學校處理後呈交報告。
學員在五個工作天後取得課程證書，才可到運輸署各牌照事務處辦理申領駕駛執照事宜。
The Transport Department will take time to review the final exam papers and submit the results to the pre-vocational schools for processing.
Trainees can only go to various driver's license service counters of the Transport Department to apply for a driver's license after obtaining the Pre-service Course Certificate five working days later.
H. 如需改期請於上課日期前48小時親臨任何門市辦理，每次改期需要支付學費的10%行政費。
If you need to reschedule, please visit any of our service counters in person at least 48 hours before the class date. Each rescheduling will require a 10% administrative fee of the tuition.
I. 上課前少於48小時更改、上課當日更改或缺席均不設改堂或者退款。
No reschedule or refunds may be made within 48 hours before class, on the day of class, or in case of absent.
J. 遲到15分鐘者作缺席論，並須重新報讀交費才可上堂。
Those who are late for 15 minutes will be absent and must re-register and pay the course fee before attending class.
K. 課程報名一經確定後, 所有已繳付之費用不設退款。
Once course registration is confirmed, all fees paid are non-refundable.

學員完成課程後，請於第5-14個工作天之內自行返回學校領取證書，逾期銷毀。日後補發,須另行收費$60。
After the completion of the course, please collect your certificate within 5-14 working days before it is destroyed. A re-issue fee of $60 will be charged for a destroyed certificate.

申請商用車輛類別的正式駕駛執照申請人必須符合申請條件，並在其遞交正式駕駛執照申請當日為有效的香港永久性居民身份證持有人，或是香港永久性居民身份證以外的香港身份證持有人，並且不受任何逗留條件所規限（但《入境條例》（第 115 章）第 2(1) 條所界定的逗留期限除外），方可獲發有關車輛類別之正式駕駛執照。

To apply for a full driving licence to drive commercial vehicle class(es), you must fulfill the application requirements and you must be the holder of a Hong Kong Permanent Identity Card, or the holder of a Hong Kong Identity Card (other than a Hong Kong Permanent Identity Card) and are not subject to any condition of stay (other than a limit of stay as defined in section 2(1)of the Immigration Ordinance (Cap. 115)

 

本人於選擇前已考慮清楚並明白所選之課程，報讀後即不能作任何更改。
l have considered carefully when making the selection and understood once confirmed, no amendment will be entertained.

本人願意遵守李健駕駛學校有限公司所訂之一切規則。
l agree to be bound by the rules and regulations of Lee Kin Driving School Ltd.
